B+A were commissioned by Hub East Central on behalf of NHS Fife to conduct a review of primary care services and premises. B+A supported NHS Fife in conducting a comprehensive review of its primary care estate portfolio, focusing on asset condition, utilisation, and alignment with future service delivery models.
The review informed strategic planning and capital prioritisation across the Health Board, with outputs used to support IJB and Scottish Government engagement.

Standardised approach
Our standardised approach ensured a successful outcome, and some of the key elements of this are outlined below. The work involved mapping estate capacity against population health needs and service transformation plans, identifying opportunities for rationalisation, investment, and co-location.
Strategic, Data-Led Assessment
We combined PAMS, local development plans, and practice-level intelligence to assess estate condition, utilisation, and future capacity needs across NHS Fife’s primary care portfolio.
Stakeholder-Driven Design
Our approach included site visits, staff engagement, and cluster-level workshops to ensure recommendations reflected the lived experience of frontline teams and aligned with service transformation goals.
Investment Prioritisation and Business Case Readiness
We produced a prioritised roadmap of short-, medium-, and long-term capital investments, ready to feed into Strategic Assessments and business case development.
Overview
A thorough review of primary care premises across NHS Fife was carried out. This has outlined significant immediate pressure is being faced by the majority of practices when seeking to find space for the new workforce within primary care (driven by the Primary Care Improvement Plan work). In addition, there are substantial housing developments across the area which in the medium and longer term will provide a further need for increased primary care accommodation.
